I may have went over this but it happend again and thought i would type it in while i rembered it.
Create a directory in your Finding_and_organizing search paths.
I used >The Bionic Woman<
then you have drive:\mediafolders\tv shows\Bionic Woman\
Then on the folder_monitor_tab Click >CHECK< it is in center about a quarter way down screen.
This then finds Bionic Woman (1976)
It showed up as flat so i clicked the box below to add a season and put in 1
The show on the download did not have the (1976) so i also filled in the custom name as just The Bionic Woman
then applied that.
It seemed to work except it still showed up as flat and when i scan with finding and organizing it did not see the files.
I also went back and did a refresh and missing file check but still did not find them.

I manually made the season 2 and 3 folder and copied some files to each then ran the rename and it did not work
so i went to my shows and deleted bionic woman off the list
then ran folder scan and all started working for it ......
It seems if you try to add a folder with out a subfolder something goes wrong.
