Folder monitor does not appear to process subdirectories where they have a particular format.
e.g. C:\TV is the root directory added to folder monitor which contains:
C:\TV\ShowA - Season 1
C:\TV\ShowA - Season 2
C:\TV\ShowA - Season 3
C:\TV\ShowA - Season 4
C:\TV\ShowB - Season 1
etc.
TVRename will try and match 'TV' against the database, and not 'ShowA' or 'ShowB'.
My workaround was to change my directory structure to one supported by rvrename.
